Deep orbital hemorrhage with vision loss is a rare complication of eyelid surgery. Following cosmetic blepharoplasty, the incidence of orbital hemorrhage after surgery has been estimated to be 1:2000 , with orbital hemorrhage resulting in permanent visual loss in 1:10,000 .21 Orbital hemorrhage usually occurs within the first 24 hours following surgery but can appear up to a week after surgery. Therefore, it is recommended that operating surgeons should remain readily available to their patients for at least 24 hours after surgery in the event of excessive postoperative bleeding.

Incision of the orbital septum and manipulation of orbital fat are likely prerequisites for blindness resulting from postoperative orbital hemorrhage. Hemorrhage within the orbit may result from traction on orbital fat, resection of orbital fat with unidentified intraoperative bleeding, or posterior extension of wound hemorrhage that often results from delayed bleeding in patients with poorly controlled systemic hypertension. Vision loss may ensue from microvascular compression that leads to ischemic optic neuropathy.22,23,24,25 It has also been postulated that a tense orbit may exceed the mean arterial pressure of the ophthalmic artery or central retinal artery resulting in central retinal artery occlusion.26,27,28,29,30,31,32 With prompt treatment vision loss can be reversible after retrobulbar hemorrhage.27,28,33,34 Rarely, severe proptosis with exposure keratopathy can also occur.

This patient had significant amounts of sagging skin affecting the upper eyelid. However, blepharoplasty alone would not be sufficient to fully deal with the problem as there was also significant brow descent causing compaction of the upper eyelid tissues. Numbness involving the upper eyelid skin and eyelashes is a common, expected outcome after upper eyelid crease incisions in blepharoplasty or blepharoptosis repair. Vertically oriented sensory nerve fibers from the supraorbital, supratrochlear, and lacrimal nerves travel in the preorbicular plane, suborbicular fascial plane, and within the orbicularis muscle.103 These distal branches of the ophthalmic division of the trigeminal nerve are transected during supratarsal eyelid crease incision for blepharoplasty and ptosis repair.104 Recovery from new nerve growth and collateral sprouting may take several weeks or months.105

Most significant eyelid scar abnormalities can be avoided by appropriate incision placement, minimal tissue manipulation, and appropriate suture selection, tension, and removal. Excessive thermal damage from electrocautery or defocusing of the laser can result in hypertrophic scarring. Asian patients and those with a history of keloid formation may be at increased risk of hypertrophic scar formation. True keloid formation in the eyelids is exceedingly rare. Local corticosteroid injections should be considered with caution due to the risk of dermal atrophy and skin depigmentation.

Sequestered epithelial remnants may form small whitish-yellow inclusion cysts along the suture line. These may atrophy in 2 to 3 months or are simply managed without anesthesia by uncapping of the cyst and marsupialization with an 18-gauge needle.

Caring For Yourself After Your Surgery

Follow these guidelines to care for yourself after your surgery.

  • Wear dark sunglasses during your trip home. You may be sensitive to light.
  • The ointment used during your surgery may make your vision blurry. This is temporary and usually goes away within 24 hours.
  • Dont wear contact lenses until at least 2 weeks after your surgery. You can wear eyeglasses.
  • Do not drive until you have stopped taking pain medication and no longer have blurry vision. Make sure that you can safely and comfortably drive a car. This is usually about 5 to 10 days after surgery.
  • Its important to get quiet, peaceful rest during the first 3 to 4 days after your surgery. To do this:
  • Avoid activities such as heavy lifting and exercise.
  • Let your healthcare provider know if youre having nausea or cold symptoms. Its important for you to avoid sneezing, coughing, and vomiting , as much as you can. These can raise your blood pressure and cause bleeding at your surgical sites.
  • Avoid bending at the waist or lifting items heavier than 5 pounds for 4 to 6 weeks after your surgery. Talk with your surgeon before going back to your usual activities, such as lifting and exercise.
  • Rest and sleep with your head raised on 2 to 3 pillows for 2 weeks or as directed by your surgeon. This will help prevent swelling at your surgical sites.
  • Your eyes may look swollen and bruised after your surgery. The eyelid swelling is usually worse early in the morning and will get better during the day as you sit or walk around.

    Try These Other Eyelid Surgery Recovery Tips

    Following eyelid surgery, your doctor will advise that you avoid makeup and stop wearing contacts for at least a week. However, most doctors recommend you wait two weeks in order to give your eyelids enough time to fully recover. Waiting two weeks will help guarantee the incision area is healed and minimize the risk of infection from applying makeup. In general, touching your eyelids as little as possible will also help reduce swelling.

    Additionally, make sure youre protecting your eyes from wind, sun, and other elements, as these can also negatively affect your recovery process. Until youre fully healed, consider wearing a big hat and sunnies, to help shield your eyes whenever youre outside.

    You might also experience redness and dry eyes after a blepharoplasty. Some doctors will prescribe eye drops, to help. If you already suffer from dry eyes, make sure to discuss that with your surgeon prior to the procedure.

    Showering And Applying Makeup

    Getting Upper Blepharoplasty (Eyelid Surgery)

    You can take showers from the neck down any time after your surgery. Dont take full showers until your surgeon tells you its safe to do so. This is usually 5 days after surgery. You may gently wash below your eyes with wipes or a soft washcloth. You can use dry shampoo for your hair. Once you can take full showers, you can wash your eyelids and shampoo your hair with mild products, such as baby shampoo.

    Dont apply creams or makeup to your surgical sites for 2 weeks after your surgery. You can use creams and makeup on the rest of your face.
