Upper Eyelid Surgery Before & After Case #9

Surgery by Dr. Baljeet K. Purewal in New Jersey.

Patient: Female Age 40-45

Concerns: She had always had heavy lids, but felt that had gotten heavier over the years leading to a tired appearance.

Treatment: Bilateral Upper Lid Blepharoplasty

What Is An Eyelid Lift Exactly

It’s an outpatient procedure that requires a very fine and small incision in the natural crease of their upper eyelid, where we remove a crescent of skin, says Dr. Sachin Shridharani, a plastic surgeon in New York City. Sometimes a slight amount of muscle and fat. The results reveal more eyelid, make the eyes look more well rested, and help improve eyesight if excess skin was blocking the field of vision. For some patients, its also about the seemingly small things like enjoying makeup.

Even though I’ve always had slightly hooded eyes, I was always able to wear eyeliner, says Suzanne Scott, a 38-year-old London-based beauty journalist who had upper blepharoplasty over the Christmas holiday. But over the last couple of years, it just felt pointless because you couldn’t see it anymore.

What Are Hooded Eyes

If you have hooded eyes, you may find that you have excess skin that hangs down over the crease. If the skin below the brow touches or comes close to your lash line, you may have hooded brows. Generally, most people are born with hooded eyes or they develop as you age. Its a completely normal and natural eyelid shape. Hooded eyelids are not medically serious and are also a normal sign of ageing. But, you can do something about hooded eyes if you prefer.

Whether youre staring at your face more through Zoom meetings or notice your hooded eyes, more people are considering eyelid surgery. If you find that your mascara or eyeliner is starting to leave a print on the brow line or disappear into the creases of the eye, hooded eyes may start to bother you.

Droopy eyelids and hooded eyes are different. Droopy eyelids are when the eyelid affects a persons ability to see. In comparison, hooded eyes tend to be hereditary or from ageing. While the two terms are often used together, they are different. Many celebrities have hooded eyes, and its not something you necessarily need to fix. Its an entirely personal choice.

Recovering From Eyelid Surgery Is A Process

9 things I wish I knew before eyelid surgery

Its natural to focus on nothing but the results after surgery, but patience is a virtue. How long does it take for eyes to heal? Think in terms of months, not weeks or days. Post-op swelling can hide the true results it may even seem like your results are asymmetrical during the initial recovery period.

Eyelid skin is the thinnest skin in the body and usually heals well and quicklywounds are water-sealed within 48 hours, and sutures often come out in seven days or less, says Dr. Tattelbaum. But how long does swelling last after eyelid surgery? It is common to see some puffiness above the incision in the upper eyelid and of the eyelash margin in the lower lid after eyelid surgery, and that can last a few months. The outer corners of the eye may have some firmness as well, he says. The good news: while you may feel these after effects, most people are unlikely to notice them. Simple things like icing and massage can go a long way toward helping tamp down swelling after eyelid surgery. We ask our patients to apply cool compresses for the first 24 hours after an eyelid lift, and then we begin a massage regimen after the first post-op visit, says Dr. Tattelbaum.

What Could Go Wrong

Eyelid surgery can occasionally result in:

  • temporary blurred or double vision
  • your eyes looking slightly uneven
  • a pool of blood collecting under the skin this usually disappears on its own after a few weeks
  • noticeable scarring

Rarely, it can result in more serious problems, including:

  • injury to eye muscles
  • the lower eyelid drooping away from the eye and turning outwards
  • the lower eyelid becoming pulled down and showing the white of the eye below the iris
  • visual impairment though this is extremely rare and many people find that their vision improves in the long term

Also, any type of operation carries a small risk of:

  • excessive bleeding
  • developing a blood clot in a vein
  • an allergic reaction to the anaesthetic

The surgeon should explain how likely these risks and complications are, and how they would be treated if they happen.

Occasionally, patients find the desired effect was not achieved and feel they need another operation.

Upper And Lower Eyelid Surgery Reduces Puffiness

This lovely lady who is in her 30s was unhappy with the puffiness in her upper and lower eyelids. Some patients have genetic excess of the fat pads around their eyes and that becomes evident at a young age, like in this lady. This patient underwent upper and lower eyelid surgery which gave her a more refreshed and yet natural result. The pictures are before surgery and 6 weeks after her surgery.

When Is A Botox Brow Lift Better Than Other Hooded Eye Remedies

A slightly droopy eyelid does not usually cause health problems, and you can easily conceal it with makeup. But patients who get eyelid lift procedures generally want makeup-free eye reshaping or rejuvenation. Besides the Botox brow lift, other treatments you can get for hooded eyes include the following:

  • Upper blepharoplasty, a type of upper eyelid cosmetic surgery
  • Laser skin resurfacing, which removes excess skin from the eyelids and enhances collagen production
  • Dermal filler injection in eye areas needing volume
  • Thread lifting, which stitches and pulls the upper eyelid skin upward
  • Radiofrequency microneedling, which stimulates collagen formation, tightening sagging eyelid skin

There are many others. However, a Botox eyelid lift has distinct advantages in certain instances

How Long Do I Have To Take Off Work After Eyelid Surgery

The exact recovery time needed varies from patient to patient, but Dr. Ford usually recommends taking at least a week off after eyelid surgery. Of course, the nature of your job will also be a factor in determining the amount of time off needed. If your occupation involves strenuous activity, you may need to extend your time off by an additional week.

How Should You Prepare For An Eyelid Lift

After your consultationduring which youll discuss your eyelid concerns and goals and ask your surgeon any questions you may haveyour doctors team will provide you with very specific pre-op instructions to help prepare you for your blepharoplasty procedure.

  • Get an eye exam to assess baseline vision and eye health
  • Have standardized before photographs taken by your surgeons team
  • Adjust your medications at least two weeks prior to surgery and stop taking certain drugs or supplements, like aspirin, ibuprofen, and fish oils, that can increase bleeding and bruising

Upper Eyelid Surgery In Leeds

An Upper Eyelid Lift or Upper Blepharoplasty is a specialist eyelid surgery procedure that removes the folds of loose skin and bulges of protruding fat which sit over the upper eyelids, to produce a less-tired and more youthful appearance.

As a result of the downward pull of gravity and loss of elasticity in the skin, the skin and muscles around the eye weaken and stretch resulting in an accumulation of loose skin and protrusion of fat resulting in eye-bags or a hooded appearance of the upper eyelid skin. This trait may run in families and may be accelerated by the effects of smoking, sun-exposure and weight-loss.

How Is Lower Eyelid Surgery Done

ST Before and AFter image

Dr. Ford prefers to perform lower eyelid surgery using general anesthesia. Because the procedure typically takes 2 hours to perform, general anesthesia is a good way to ensure patient comfort and safety. Dr. Ford makes a long, thin incision just below the lower lash line. He manipulates the underlying muscle, raising it to create a cleaner and more alert appearance. Often, he also raises the cheek tissue, blending the lower eyelid with the cheek for a more natural-looking delineation. He removes any excess skin and closes the incision with sutures. He also uses sutures to splint the outer upper and lower lids together. All of these sutures are removed 7 days after surgery.

What Happens During An Eyelid Surgery Procedure

Upper Blepharoplasty

For cosmetic surgery on your upper eyelids, you will receive local anesthesia, to numb the area. Markings will be made prior to upper bleph surgery with you sitting up, to determine the exact amount of skin to be removed.

Once youre numb, your blepharoplasty surgeon will make an incision along the natural crease in your eyelid and remove any excess skin. Occasionally, a small amount of upper eyelid fat is removed as well.

It basically involves cutting and suturing the eyelid back together, says Dr. Kapadia. Most of the time, were just removing extra eyelid skin, but sometimes well also do a muscle tuck, to make the eye appear more open, he says.

Both upper and lower eyelid surgeries can be performed with a scalpel or a laser. When used correctly, the two tools yield similar resultsits a matter of surgeon preference. Doctors who favor a laser say that theres less bleeding, while others claim that the incisions take longer to heal.

Lower Blepharoplasty

Lower eyelid surgery can be a bit more uncomfortable for a patient, so Dr. Kapadia performs it under intravenous sedation or general anesthesia.

This procedure can remove or reposition extra eyelid fat, tone down puffiness, and tighten lax or sagging skin.

If your surgeon will need to remove excess skin, the incision is usually made just below or behind the lower lash line.

Patient 3 Before And Two Months After Upper Blepharoplasty Surgery

These photos show a woman in her late 40s before and after upper blepharoplasty surgery. This patient is younger than the patients shown above and has less excess eyelid skin, but you can still notice a big improvement from surgery. More skin was removed from the patients right side than the left to improve symmetry.

Removing the excess skin helps frame her eyes in a better way and highlights the beauty of her stunning blue eyes. The beauty of the eyes can be further enhanced with eye makeup when desired. Prior to surgery, makeup would have been covered by the extra skin.

Eyelid Surgery To Remove Excess Skin From Hooded Eyelids

My Eye Lift Plastic Surgery – Before & after blepharoplasty

Sometimes referred to as an eyelift or eyelid lift, blepharoplasty is meant to reshape the upper eyelid and lower eyelids appearance and remove bags. In most cases, a board-certified cosmetic surgeon will perform the surgery on both the upper and lower eyelids. But they can also perform the surgery on just the upper eyelid or lower eyelid exclusively.

Your surgeon will remove excess skin and tissue from the eyelid area and adjust the underlying eye muscles to treat droopy eyelids. The end result is the removal of bagging and puffiness from the eyes and a smoother eyelid area. Patients who undergo hooded eyelid surgery appear more alert, awake, and younger.

Who Is A Good Candidate For Hooded Eye Surgery

You are a good candidate for eyelid surgery if you have excess upper eyelid skin. The excess skin restricts ones visual field, causing heaviness in the eyelids and chronic tension headaches. Hooded eyelids and droopy eyelids give one a tired and sleepy look. Suitable candidates for eyelid surgery are generally healthy and non-smokers.

Shirazi explains that the loss of structure around the eyes can cause the eyebrows to droop. Adding a dermal filler to the area can provide a lift to the eyebrows and decrease the look of hooded eyelids.

Is Eyelid Surgery Dangerous

Hooded Eye Surgery Before And After

Blepharoplasty surgery is considered to be safe, with minimal side effects. However, like any surgical procedure, it comes with risks and possible complications, including a reaction to the anesthesia or infection.

Risks and side effects associated with both upper and lower blepharoplasty include:

  • swelling, including chemosis
  • temporarily blurred vision ,

You may also have watery eyes, double vision, light sensitivity, or discomfort during your recovery.

According to a 2013 study in the Journal of American Medicine Association Facial Plastic Surgery, dry eye problems can develop or become more noticeable after an eyelid lift. This can be treated with medicated eye drops.

It’s important to have your eyes tested prior to surgery to detect any pre-existing dry eye. This wouldnt mean you cant undergo surgery, but you should start using eye drops prior to surgery, and your eyelid surgeon may take a more conservative approach.

One of the most rare but difficult to correct blepharoplasty complications is having too much skin removed, which can make it difficult to fully close your eyes. Going to an experienced surgeon who specializes in eyelid lifts will help minimize this risk.

To understand all potential risks, talk with your surgeon and disclose any medical conditions you have. Even in the most experienced hands, surgical complications are possible, though highly unlikely, explains Dr. Thomas J. Walker, a plastic surgeon in Atlanta.

Upper Blepharoplasty Vs Brow Lift: Which Is Right For You

Its fairly common for patients to ask whether they are better candidates for a brow lift or upper eyelid surgery. Thats because the cosmetic concernsfrown lines and a heaviness around the eyesare similar for both procedures.

Dr. Burke evaluates the position of the eyebrows to help determine which procedure is best suited for you. If the eyebrows remain in a relatively good position, eyelid surgery to remove excess skin is often recommended. When the brows have lowered, though, a brow lift may be a better approach. Some patients may benefit from a combined procedure.

Are Hooded Eyelids Uncomfortable

  • The extra layers of skin that droop down can be heavy on the eyes and cause them discomfort. They can also and make it harder for you to see making you constantly tilt your head backwards even during a conversation. Moreover, saggy eyelids can make your eyes dry which can cause irritation and discomfort.
