To prepare for surgery, patients may be asked to do the following:

  • Tell their physician about all the medications being taken, including vitamins and supplements.
  • Stop taking medications that can interfere with the bloods ability to clot.
  • Refrain from eating or drinking 6 to 12 hours before the procedure.

The day of surgery, patients will need to arrange for someone to drive them home afterward, since it is an outpatient procedure. The surgery usually takes about 30 minutes.

What Causes Dequervains Tenosynovitis

DeQuervains Tenosynovitis in most people is a repetitive strain injury. Most cases of DeQuervains result from chronic overuse of the wrist, especially in jobs that require repeated use of the hand and wrist. It is caused by frequent wringing actions and movements that load the wrist while it is held in slight flexion and tilted towards the body. The most common cause of this in the past was washing and wringing clothes, giving rise to it being called washerwomans sprain. These days, the most common cause is carrying a baby and pumping milk for breastfeeding, and it is now commonly known as mummy thumb.

Other causes are direct injury to the wrist resulting in scar tissue that restricts tendon movements, inflammatory arthritis, or poor hand posture when typing and using a mouse for long hours daily. Doing housework with a lot of wringing and scrubbing also can cause it. You may not associate it with these activities, because the thickening builds up slowly over time until it is bad enough to notice. Usually this takes years or decades, although occasionally it can happen soon after a twisting sprain to the wrist or after doing a lot of housework.

It is most common in first time mothers or grandmothers, and in people between 40-60 years of age. It may also appear in the late stage of pregnancy due to hormonal changes and water retention.

Can De Quervains Tenosynovitis Be Prevented Or Avoided

The best way to prevent de Quervains tenosynovitis is by avoiding repetitive movements. Change your actions to reduce the stress on your wrists. Take frequent breaks to rest if you are using your wrists. Wear a brace or splint on your thumb and wrist, if necessary. Follow the exercise routine suggested by your doctor or physical therapist. Be sure to tell them about any activities that cause pain, numbness, or swelling.

Is There Anything Else That Can Help Calm It Down

Some people will find the use of a thumb splint can be useful to help calm their pain down. They can be issued by the physiotherapy department or can be bought easily online .

You should be aware that the splint should only be used temporarily, and as soon as your thumb starts to feel better, you should start to reduce how much you use it. Although splints can be useful, they can sometimes be tempting to use all the time or carry on using just in case. Remember, your thumb and wrist are meant to be used and therefore, the way to get the best overall recovery is to calm it down and then build it back up, rather than simply rely on a splint.

How Do I Know I Have It

In the early stages, you may just have some pain in the wrist near the base of the thumb off and on, especially with thumb movements or wrist movements. Most people will ignore this and carry on with their activities as normal.

As it gets worse, you will notice that the pain in the wrist is more frequent and affects simple activities like brushing your teeth, opening doors or bottles, and reaching behind the back. Typing or using the mouse may also be painful. You may notice some swelling at the side of the wrist below the thumb.

De Quervain’s Surgery

Treatment for de Quervains tenosynovitis focuses on reducing pain and swelling. It includes:

  • Applying heat or ice to the affected area
  • Taking a nonsteroidal anti-inflammatory drug . These include ibuprofen or naproxen
  • Avoiding activities that cause pain and swelling. Especially avoid those that involve repetitive hand and wrist motions.
  • Wearing a splint 24 hours a day for 4 to 6 weeks to rest your thumb and wrist
  • Getting injections of steroids or a local anesthetic into the tendon sheath. These injections are very effective and are used regularly.

A physical therapist or occupational therapist can show you how to change the way you move. This can reduce stress on your wrist. They can also teach you exercises to strengthen your muscles.

Most people notice improvement after 4 to 6 weeks of treatment. They are able to use their hands and wrists without pain once the swelling is gone.

Rehabilitation Post De Quervains Release

The hand surgeon will immobilise your wrist using a cast for 10-14 days to allow the wound and soft tissues to heal. Immediately post hand surgery it is important to keep the hand elevated, the cast dry and to gently exercise the thumb to keep the tendons gliding through the recently released tunnel.

The hand and wrist doctor will then remove the cast, then check the wound and a light dressing reapplied. A wrist and thumb splint is fabricated to wear for the following two weeks. This splint must be removed five times a day for thumb and wrist exercises, but provides support and pain relief when using the hand. Therapy will include swelling and scar management, wrist and thumb range of movement and strengthening.

The thumb and wrist will likely still be recovering at six weeks post-surgery but should be well healed after three months.

What To Expect During Surgical Release

Surgical release of the tendon will reduce the pressure in your wrist. The doctor makes an incision in your wrist near the base of your thumb. Then the surgeon opens the tissue or sheaths over the swollen tendons to relieve the tendon and the compressed nerves. Loosening them up reduces the pain. During rehabilitation, you’ll possibly feel numb or tingling near the surgical site. You’ll go through physical or occupational therapy. At the same time, you’ll wear a splint on your hand for a month after surgery.

You should definitely avoid any heavy lifting or repetitive motions immediately following De Quervains surgery. It may take 6 to 12 weeks for your hand to heal completely. You should then be able to move your wrist and hand freely without any pain. Of course, everyones journey to recovery varies.

What Should I Expect With These Exercises Will They Be Painful

This is a very important question.

If your wrist has been sore for some time, it would be a bit unrealistic to expect that it will be pain-free when doing these exercises. However it is important that you dont do too much to then upset the problem all over again.

A good marker for how hard to work is that you should feel some discomfort , but it should settle within 24 hours. If you are still obviously sore the following day, you may have done too much/ pushed too hard, therefore next time you do the exercises, reduce the effort a bit.

How Do I Prevent It

The way to prevent DeQuervains is to avoid or minimise activities requiring frequent or strong wringing or repetitive side to side wrist movements.

This means avoiding wringing or squeezing actions such as squeezing clothes or towels dry and carrying a baby by hand. Hang your clothes or towels to dry without squeezing first, or if you must, use a gentle squeeze only. Using a baby carrier and breast-feeding pillow will help you carry the baby without straining the hand and wrist.

Pay strict attention to your hand posture when using the keyboard and mouse and take a break every 30-60 minutes to stretch the fingers, thumb and wrist.

The time to recover from De Quervains Tendonitiss surgery is about three to five weeks for most patients. Following surgery a dressing is left on the wrist until the sutures are removed. Patients may start using their thumb immediately and will gradually gain back their range of motion and strength over the several weeks. While simple activities begin almost immediately after surgery, strenuous activities can be uncomfortable for a few weeks.

Hand therapy exercises are often recommended to help facilitate a return of strength and range of motion. De Quervains release is a small surgery. Most patients regain their motion without needing physical therapy.

After recovering from surgery and successfully completing their rehabilitation plan the patient should be able to use their hand normally without pain. Once completely healed, patients should be able to return to all previous daily activities without any restrictions.

What Are The Symptoms Of De Quervain’s Tenosynovitis
