Long Term Diet After Gastric Sleeve Surgery

Stage : Add Thicker Drinks & Smooth Foods

Gastric Sleeve Diet | Post-Operation

Begins 1 day post-op to 14 days post-op

Your surgeon will confirm that you have no nausea or vomiting before allowing you to begin Stage 2 of your post-op gastric sleeve diet. Stage 2 lasts from one day to 2 weeks, depending on your surgeon, and includes thicker drinks, pureed foods, andsmall meals:

  • Clear liquids list from Stage 1

Remember: No soft or solid food and no drinks with chunks or seeds.

You can also drink clear liquids between meals.

While drinking anything:

  • Stop immediately as soon as you feel full or feel any pressure

Protein 1 Year After Gastric Sleeve

RNY and Sleeve patients should get at least 60 grams and 75 grams of protein per day.

BPD/DS patients should get more, at least 90 grams of protein per day

When weight loss is the goal there are only 2 things that you need to focus on: Total Calories Consumed and Total Protein Consumed.

If you get those two numbers correct 90% of the time then you will lose weight.

Why is protein so important?

During times of calorie restriction , its been proven that higher protein intake is helpful in maintaining lean body mass.

After all, you want to lose weight from body fat, not lean body mass.

Further, protein is the most fillingmacronutrient, and hunger is arguably the most uncomfortable part about dieting for weight loss. Swap out your carb-laden snack for a comparable protein snack and you can fend off hunger until dinner much, much easier.

Gastric Bypass Liquid Diet

Your 2 or 3 day liquid diet before gastric bypass is the most important part of your pre surgery preparation. This diet will be low in fat and sugars, but will still fulfill your daily nutritional requirement of about 100 grams of protein and about 1000 calories. The actual diet composition will be customized according to your unique needs.

Protein shakes are a key element in your clear liquid diet before gastric bypass surgery. You should choose shakes that contain at least 15 grams of protein per serving and not more than 5 grams of sugar. A range of other liquids may also form a part of your gastric bypass liquid diet, including hot cereals and clear soups.

Stage 2 Only Puree Foods For 2 Weeks

All food during this stage should be pureed and completely smooth with no bits or lumps.

It is important to complete this stage as your stomach pouch will be very small and tight at first. Puree foods are smooth and can pass through your stomach easily. You will need to use a blender, liquidiser or food processor to puree your food to the correct consistency. Aim for a smooth consistency like yoghurt.

Choose foods that are high in protein to help you to meet your protein requirements . Aiming for at least 500ml of protein rich fluids daily will help you achieve this.

Slippage or pouch dilation occurs in 12% of patients.25,26 Symptoms include epigastric pain, nausea, and vomiting. Although this complication usually is unavoidable, eating slowly and not overfilling the gastric pouch may help prevent it. If the patient is in extremis, early operation is required to avoid gastric resection for ischemia or perforation of the slipped segment. Symptoms usually are not so severe, and we are generally able to perform band repositioning or removal.

Esophageal dilation occurs in 2% of patients. It is usually insidious with late onset of inability to tolerate food. The cause is unknown. Esophageal dilation is treated by band deflation or by removal of the band if dilation is severe.

Erosion of the band into the stomach occurs in less than 1% of patients. Symptoms include lack of restriction, latent port infection, and dysphagia or epigastric pain. The patient may also be asymptomatic. It is identified by esophagogastroduodenoscopy and is usually missed on upper gastrointestinal series. This complication requires removal of the band and port.

Port complications occur in 7% of patients. There are several types of complications. Most common is a leak in the tubing or port itself, leading to inability to adjust the band and to loss of restriction to eating. Less common is port dislodgment from the muscle fascia, making it difficult to adjust the band. Treatment comprises port replacement or repositioning.

Etiology Of Thiamin And Folate Deficiencies And Recommended Supplements

Thiamin deficiency is caused by inadequate dietary intake, as primary absorption sites may be bypassed.23 Clinical presentations have included acute Wernicke encephalopathy , lower limb hypotonia, seizures, polyneuropathy, unsteady gait and ataxia, and hearing loss. Severe deficiency is associated with beriberi. Dry beriberi is the development of a symmetric peripheral neuropathy characterized by sensory and motor impairments mostly of the distal extremities, as demonstrated by difficulty in rising from a squatting position. Wet beriberi manifests as cardiac impairment with peripheral vasodilation, cardiomyopathy, congestive heart failure, edema, tachycardia, peripheral neuritis, and Wernicke-Korsakoff syndrome. Diagnosis can be made by measuring erythrocyte transketolase activity, blood thiamine concentration, or transketolase urinary thiamine excretion. Patients should receive daily B-complex supplements to prevent deficiency. Treatment for acute deficiency manifested by cardiovascular or neurologic signs involves administration of supplemental thiamine, starting with 100 mg/d intravenously for 7 days, followed by 10 mg/d orally until there is complete recovery.

The cause of folate deficiency is unknown. Deficiency is usually prevented if the individual takes a daily multivitamin.

Fortunately, the basics are all you need in order to enjoy steady and safe weight loss after bariatric surgery.

Like I said a moment ago your diet 1 year after bariatric surgery should look like the diet you were following when you were losing weight after surgery. That is, the last phase of your 4 Phase Post-op Dietthe basics.

Avoid Eating And Drinking Simultaneously

Eating and drinking at once are not recommended in this diet. The capacity of your stomach is limited after bariatric surgery, which is why drinking during meals should be avoided. Drinking while eating will quickly fill up your stomach with fluid, and you wonât get sufficient nutrients.

Focus on drinking fluids thirty minutes before and thirty minutes after the meals. Having too much liquid before or right after your meals would also give the floating food sensation, resulting in nausea.

Three key guidelines for this phase of your post op diet will include: Sip 48 to 64oz of liquids per day to stay hydrated Drink fruit and vegetable juices in diluted form to avoid diarrhea or nausea and Avoid drinking extremely cold or hot liquids.

Gastric Bypass Diet At A Glance


After the gastric bypass has been performed, you will begin the first phase of your diet. Water is the best way to do this, and you will need to drink at least 96 ounces every day to prevent dehydration. Because you wont be getting any additional hydration from foods, its very important that you reach this goal every day.

This will help you feel full and along with the protein supplements keep your body nourished. If water is just too bland for you, consider throwing in a slice of lemon or another citrus fruit to add a little flavor. Talk to your dietitian about drinking protein shakes to meet your protein requirement.

Dont gulp the water, but rather take little sips throughout the day. Remember, your stomach is much smaller now and wont be able to handle too much. Avoid liquids high in sugar and stay away from carbonated drinks that may upset the stomach.

As the weeks turn to months, soft foods and then solid foods will be introduced to the diet. By this time, hormonal changes in your body will diminish the number of hunger pangs you feel and youll adapt to the smaller intake of food.

Get Plenty Of The Right Kind Of Fluids

Ensuring that youre getting enough fluids is another important aspect of post-surgical living. Healthcare providers recommend that you get about 40 ounces of liquid, such as water or clear juices, a day.

Youll have to steer clear of coffee or tea for the first two months following surgery, and you should abstain from alcohol for at least a year. Stage 4 Regular Foods From Week 7 Onwards

You will now have completed 6 weeks of texture progression and your stomach should have healed enough for you to gradually introduce regular texture foods and begin to increase the variety of foods in your diet.

Continue to eat 3 regular meals each day and try not to leave more than 4-5 hours between meals. You should not graze or go for a long-period of time without eating.

Use a side plate to help guide your portions and follow the plate model below, so that you are always prioritising protein first. If you are hungry in-between meals then choose a nutritious snack such as a piece of fruit, high protein yoghurt or a glass of skimmed milk.



What Foods Should I Eat


Youll notice that if you search for most of these food items that your trip to the grocery store will keep you to the perimeter sections of the store. These areas typically have more fresh and unprocessed items, which is a good thing.

Stick to whole food options and supplement with high-quality protein supplements for the sole purpose of helping you reach your daily protein goal.

    Recommended Meal Plan For Weeks Two To Eight Until Two Months Post

    At this time, your caloric intake will probably be no more than 500 calories a day, divided into six to eight small meals. Recommended portion sizes are 1/4 cup for solids and 1/2 cup for liquids.

    This sample menu includes different foods that are safe for you to eat. You may adjust the menu to fit your tastes and tolerance.

    Breakfast1/4 cup hot cereal made with non-fat milk


    1/2 cup low-fat chicken noodle soup

    Late Afternoon1/4 cup sugar-free, nonfat yogurt

    Remember to drink 1 cup of water or other non-caloric fluids between meals. It is important to take a multivitamin and mineral supplement every day, plus additional iron if required, and calcium and vitamin D supplements two to three times per day.

    What Shall I Do If Food Gets Stuck

    If this happens it may be because you have eaten a food that was too solid, eaten too quickly or too much, or because of eating certain problem foods. This will feel uncomfortable and you may wretch or vomit. Often it will resolve itself, however you could try drinking a small amount of cold sparkling water, as the fizz may help to dislodge the food that is stuck.

    What Else Can You Expect After Surgery

    Side effects

    After your surgery vomiting can occur. It is often the result of:

    • Eating too much
    • Not chewing your food well enough
    • Eating and drinking together, or too closely together
    • Eating foods that are not the correct consistency for the stage you are in
    • If you continue to vomit after checking the above and you are still within the first 2 months after surgery, you could try returning to the puree or liquid phase for a short period to see if your symptoms improve.
    • If you continue to vomit and the cause cannot be explained then please contact your surgeon or GP. Very occasionally there may be a narrowing where the new joins in your intestine have been created and this may require a procedure to stretch the area, so that food can move through.
