Both 2020 and 2021 were pivotal years for facial plastic surgery. Despite the national pause on elective procedures in 2020, three of the top five most performed surgeries were on the face, according to data from the American Society of Plastic Surgeons. In terms of advancements in surgical techniques, there are procedures being developed to drive the native facial muscles with the masseteric branch of the trigeminal nerve with minimal injuries to the donor site, as well as studies on using a cross-facial nerve grafts for the reanimation of the paralyzed face.115116117118Cross-facial nerve grafts utilize the contralateral healthy facial nerve to reinnervate the paralyzed muscles.119120

Cosmetic surgery is unlikely to change your life. Most board-certified plastic surgeons spend a lot of time interviewing teens who want plastic surgery to decide if they are good candidates for the surgery. Doctors want to know that teens are emotionally mature enough to handle the surgery and that they’re doing it for the right reasons.

Many plastic surgery procedures are just that surgery. They involve anesthesia, wound healing, and other serious risks. Doctors who perform these procedures want to know that their patients are capable of understanding and handling the stress of surgery.

Some doctors won’t perform certain procedures on a teen until they are sure that person is old enough and has finished growing. For rhinoplasty, that means about 15 or 16 for girls and about a year older for guys.

Girls who want to enlarge their breasts for cosmetic reasons usually must be at least 18 because saline implants are only approved for women 18 and older. Forehead lifts or browlifts also are a large part of a facial plastic surgery practice. This procedure is relatively straightforward in terms of its classical approach where excess skin is resected and the forehead skin is repositioned superiorly.31The incisions are placed along a coronal line within the hair-bearing scalp if the frontal hairline is low. A hairline incision is used in patients with a high hairline.32These two approaches are most commonly used in women. In men, a third option is the midbrowlift where an incision is made in a deep brow furrow, and a fusiform ellipse of skin is resected.32A direct browlift, where the incision is made at the upper margin of the eyebrows, is rarely performed. Both mid- and direct browlifts may leave a visible scar, and they are more commonly used for functional brow surgery in patients who have significant brow ptosis contributing to a mechanical visual field defect. The newest approach is endoscopic surgery where several small incisions are placed behind the hairline and an endoscope is used for visualization during elevation of the forehead skin. With the endoscope under the forehead flap, the surgeon releases soft tissue from the arcus marginalis and temporal line of fusion, allowing redraping and fixation of the skin more cephalically.3334The results are less dramatic, but it is an outstanding approach in the younger patient who seeks more natural and less-defined changes.
