FTM Top Surgery, or Chest Masculinization, is a gender-affirming procedure that involves the removal of the breasts and male chest reconstruction to create a masculine-looking chest. Top Surgery is the most commonly performed gender-affirming surgery for trans masculine people and may be the only surgical step taken in a female-to-male transition. Studies have shown that Top Surgery improves psychological and social functioning, helping trans men to live more comfortably.

Recovering From Top Surgery

It is very important to follow all the instructions your surgery team gives you when you go home from the hospital. For several weeks after surgery you should not lift your arms greater than 90 degrees away from your body or over your head. You should also not push, pull or lift anything greater than five pounds during these four weeks. This ensures your scar will heal well and not stretch and become larger.

Pain. You may experience some discomfort for a few days after surgery, but generally after four to five days your pain will ease and can be managed with over-the-counter acetaminophen and ibuprofen.

Dressings and drains. You will have dressings and possibly drains in place when you leave the hospital. Do not remove them until you see your doctor after surgery. If you have drains, your doctors office will give you instructions on how to empty them. These drains help prevent fluid from collecting in the chest or breast. Please record the amount of fluid coming out of the drains. Your drains will usually be removed at your first appointment after surgery.

Bathing and showering. Until you see the doctor, you may shower your lower body if you can do so without getting the bandages and dressings wet. Clean the top of your body with a sponge only until your doctor gives you the go-ahead for showering and bathing.

Mtf/mtn Top Surgery Procedure

MTF and MTN top surgery is also known as breast augmentation or augmentation mammoplasty. MTF and MTN top surgery typically takes between 1 and 2 hours. You are put under general anesthesia for the duration of the surgery. The breast augmentation procedure your surgeon uses will vary based on your desired chest size, the type of implant used, and the incision location. Key information includes:

  • Youll typically have a choice between saline implants or silicone implants .
  • Silicone implants have the reputation of being softer and more natural appearing while saline implants are often less costly and can be inserted through a smaller incision.
  • Incisions are most commonly located along the areola, under the armpit, or under the fold of skin where your chest meets your breast tissue.
  • Once under general anesthesia, the surgeon makes an incision in the previously determined location and the implant is placed in the pocket above or below the pectoral muscle.

All Surgery Carries Some Uncertainty And Risk

Top surgery is not a simple operation, but its normally safe when performed by a qualified plastic surgeon. Nevertheless, as with any surgery, there is always a possibility of complications, including bleeding, infection, or reaction to the anesthesia. You can reduce your risks by closely following your physicians advice both before and after surgery.

The procedure does leave noticeable, permanent scars. The scars fade with time, but never disappear completely. Poor healing and less favorable scars are more common in smokers. Though efforts are made to make the two sides of your chest as symmetric as possible, the procedure also can leave you with slightly mismatched chest walls or unevenly positioned nipples. Most patients experience a permanent change in feeling in their nipples and chest wall.

What About Dog Ears


I hate dog ears just as much as you. I do my best to avoid them. I also hate scars, and so I might leave a little dog ear at the end of the incision to shrink some before I finally remove it. This will help minimize your final scar length. Sometimes preventing large dog ears will involve extra cost to include liposuction or to extend the incision off the breast and into the armpits. Sometimes it is simply not possible to completely remove a dog ear at the time of the first operation. Typically I let you heal and then finish the dog ear removal in the office

When Can You Lift Heavy After Top Surgery

This depends on what you consider heavy, what type of top surgery you had, and first and foremost what your surgeon says. If youve never trained before and an empty 45-pound barbell is heavy, its amazing that you want to get started. After an initial five or six weeks of smooth recovery, check in with your surgeon about building up to lifting the barbell.

If you have more experience and consider 135 pounds or more to be heavy, build up in a similar way but youll have to wait a little longer. The three month mark is when your surgeon is more likely to clear you to start lifting more normally if youre experienced. In the long run, waiting an extra two weeks to go heavy will be easier than hurting yourself and jeopardizing your results. Go slowly.

To maximize your results, minimize your complications, and have the best scars, just plan to take some time off to recover and set the next part of your life up for success.

About Masculinizing Top Surgery

Also called subcutaneous mastectomy, male/masculinizing chest contouring.

Dr. Esther Kim performs this procedure in the Division of Plastic Surgery at UCSF. Masculinizing Top surgery will shape the skin and tissue of your chest to match the contour of a male chest. There are many factors that Dr. Kim will discuss with you during your consultation. These factors include your current shape and size, body type, skin type, and nipple size and position. Dr. Kim is a proponent of double incision surgery with free nipple grafts. Most transmasculine patients with larger breasts are good candidates for a double incision chest surgery with free nipple grafts, where the lower scars are hidden under the lower border of the pectoralis muscle in the male chest. Depending on your chest dimensions, the nipple and areola is resized and new nipple positions are chosen to look more masculine. Patient with smaller breasts may also be candidate for either a periareolar incision subcutaneous mastectomy . During your consultation, Dr. Kim will discuss what options are available and your expected postoperative result.

Who May Benefit From Top Surgery

Transgender and nonbinary people may choose top surgery as part of their gender affirmation treatment. Gender affirmation surgery can address gender dysphoria, which occurs when gender identity does not correspond to sex assigned at birth.

Gender affirmation surgery can improve well-being in patients who:

  • Have ongoing and documented gender dysphoria.
  • Have the legal and mental ability to make responsible medical decisions for themselves.
  • Are successfully addressing other mental and physical health concerns.

While testosterone therapy is not required for chest masculinization surgery, some surgeons and insurance companies will request one year of estrogen therapy to maximize natural breast growth prior to implant placement.

If youre new to lifting in general, that means youll likely be lifting lighter weights than you would if you were more experienced. For the first few weeks, surgeons often advise that people of any lifting experience should avoid picking up anything heavier than five pounds. Your body may then be ready to start lifting more weights around 20 pounds around five to six weeks after surgery. Start very slowly, even if youre feeling confident. Better to go slower and heal well than to move faster and put your recovery at risk.

Even for experienced lifters, this might mean performing push-ups from your knees for the first few sessions. How To Prepare For The Operation:

    Why Should I Do Prehab

    How To Maintain Shoulder Mobility During Top Surgery Recovery

    Many surgeons advise that you not raise your hands above your head for two to three months after surgery to avoid your scars stretching and becoming wider. But even during that eight to 12-week window, you can take some action to maintain or build some level of mobility to avoid stiffness and pain.

    Dr. Ginger Slack, M.D., an Assistant Clinical Professor in the UCLA Division of Plastic Surgery, told BarBend in an email that it helps to focus on your range of motion between zero and 90 degrees during recovery. Since you cant lift your hands above your head for a while, she encourages patients to follow their surgeons specific advice about maintaining mobility between 0 and 90 degrees.

    To do this, try bending your elbows and raising them slightly above your shoulders, with your hands pointed at the ceiling. Exhale and move your elbows out to face the sides of the room, then move back. Perform these shoulder sweeps for three sets of 10 to 15 repetitions once or twice a day after youve gotten your drains out . Itll help remind you to stand up as straight as you can. It might produce a small stretching sensation in your chest, which is common given that youre likely to be hunched over protectively during your early recovery .

    What About Smoking Or Using Nicotine

    Patients currently using nicotine are not candidates for surgery. Blood testing for nicotine is frequently performed if there is any doubt. Using nicotine in any form is extremely detrimental to wound healing and can seriously impact your surgical result in a very negative fashion. You must be off nicotine for at least 6 weeks prior to the surgery and promise to remain off it for at least 6 weeks after to qualify.

    What About Breast Cancer

    Most top surgeries, in order to avoid a scooped out chest appearance, will need to leave some breast tissue behind. In this way, the operation should be thought of more as a breast reduction than a true mastectomy. There will still be a risk for breast cancer, although diminished somewhat following the operation. Weekly breast self-examination should still be performed, and you should inform your primary doctor that you are still at risk of breast cancer to keep it on their radar.     When should you begin prehab? A good rule of thumb is to start at least 4-6 weeks prior to your surgery date. Just like rehab after surgery, youll want to attend prehab therapy on a regular basis, at least 2 or 3 times a week. Also just like post-op PT, in addition to attending prehab sessions at a clinic, youll want to perform the recommended daily exercises at home, as needed.

    Will Insurance Pay For The Surgery

    We accept a wide range of insurance plans at Larson Plastic Surgery. Each insurance policy is different therefore, I cant say for certain if your insurance provider will provide coverage for the operation. They will frequently provide coverage for the office consultation. Arizona Medicaid plans and AHCCCs do not cover the surgery. My best advice is to look at the fine print of your policy or call your insurance company and ask. Occasionally, your insurance will cover part of the operation, the mastectomy without covering the nipple reconstruction.
