IOLs introduced into the US market during the last 2 years offer cataract surgeons an almost dizzying array of options for pseudophakic correction, said Priyanka Sood, MD.

Sood described the technologies and offered insights shared with her by colleagues who had experience with the various lenses.

At bottom, there are pros and cons to consider for each IOL, and achieving patient satisfaction depends on careful selection and thorough preoperative counseling.

The reality is that none of these lenses is the holy grail able to restore prepresbyopic quality vision, said Sood, an assistant professor of ophthalmology at Emory University in Atlanta, Georgia.

Monofocal IOLsThe monofocal category was expanded recently with 2 novel aspheric hydrophobic acrylic lensesthe TECNIS Eyhance and the Clareon . The Eyhance IOL has extended depth of focus -like qualities through the design of the central 1 mm of its optic. However, the Eyhance was not approved as an EDoF lens and surgeons cannot charge a premium fee for it. Outside the central 1 mm, the Eyhance optic is similar to the monofocal ZCB00 , which explains its good contrast sensitivity, Sood said.

Katie Petro, MD, told Sood she has had good results with the Eyhance and that patients who have received implants are happy.

What Makes The Design Of The Vivity Iol Different

One of the side effects of many Multifocal IOLs is glare and halo symptoms. The benefit of many Multifocal IOLs is achieved by diffracting light. Essentially this means scattering light in a controlled way. As light enters the eye diffractive MFIOLs split this light into distance and near focal points for distance and near vision. As a side effect, some of this scattered light is perceived as glare, halos and a reduction in the sharpness of vision. Different MFIOLs cause different amounts of glare and halo symptoms. If you are interested in MFIOLs, this is a conversation to have with your surgeon.

The Vivity was designed to reduce glare and halo symptoms as much as possible compared to other MFIOLs on the market. This lens may be an option for diabetics with cataracts, however, this should be based on a discussion with your surgeon, since there is a broad spectrum of severity of diabetic eye diseases.

As a trade-off, the Vivity does not provide as close of near vision as some other MFIOLs. The Vivity is targeted more for patients interested in the intermediate range of vision , which would be more suited for computer and dashboard range vision than near reading of small print.
