Michael, 17, arrived in the sleek white waiting room of his plastic surgeons office in Miami for a moment he had long anticipated: removing the bandages to see his newly flat chest.

Republican elected officials across the United States are seeking to ban all so-called gender-affirming care for minors, turning an intensely personal medical decision into a political maelstrom with significant consequences for transgender adolescents and their families.

I cant honestly think of another field where the volume has exploded like that, said Dr. Karen Yokoo, a retired plastic surgeon at the hospital.

How Do We Identify Frailty And How Does It Affect Health

There are many different tools we can use to detect frailty. The Clinical Frailty Scale is one tool based on clinical features present in the patient and the Frailty Index is another tool based on the accumulation of deficits in the patient.

The Clinical Frailty Scale is a single descriptor of a persons level of frailty using clinical judgement graded from one to nine. Level one is a very fit person level four is vulnerable where the person is not dependent on others for help with daily activities but does have symptoms that limit activities and level nine is a terminally ill person.

It has been observed that people with a higher Clinical Frailty Scale were more likely to be older, female, have a degree of cognitive impairment and incontinence. The higher proportion of females will most likely reflect the longer life expectancy of women.

Frail people have a higher risk of recurrent falls and fractures and subsequent disability and reduced function. There have been many studies performed to examine how well frailty predicts outcomes after surgery.

In people who have surgery, frailty has been shown to be associated with a higher risk of surgical complications, a greater chance of requiring discharge to a residential care facility and a lower rate of survival. And the more frail the patient, the higher the risk the patient will require readmission after surgery, and the higher the risk of death.

It is very important to follow all the instructions your surgery team gives you when you go home from the hospital. Bathing and showering. Until you see the doctor, you may shower your lower body if you can do so without getting the bandages and dressings wet. Clean the top of your body with a sponge only until your doctor gives you the go-ahead for showering and bathing.

Its encouraged that pain medication is used when needed, but patients typically find that pain during recovery is very minimal and subsides after a couple of days. That means if you are in your 40s, 50s or even older, you do have the opportunity to undergo gender affirming surgery. Of course, you must meet all the criteria for doing so which is true for all ages. That includes obtaining your letter from a mental health expert affirming gender dysphoria and supporting your desire for the surgery. Issues of general health can play more of a role at an older age versus a much younger age. However, if you are in good health, then you should be good to go.

Gender surgery for trans women includes:

  • removal of the testes
  • construction of a vulva
  • construction of a clitoris

Breast implants for trans women are not routinely available on the NHS.

Facial feminisation surgery and hair transplants are not routinely available on the NHS.

Trans individuals most often have top surgery after they are 18. However, for some FtM patients, younger individuals may be considered for top surgery if their legal guardians and mental health providers believe the procedure is appropriate to their health and safety. The maturity level of the patient must be considered as well in such instances. Male chest reconstruction isnt recommended for transmasculine or non-binary individuals with future plans to breastfeed, as the procedure interferes with the proper functioning of the milk ducts.

The situation is a bit different for trans women. Breasts will grow naturally for up to 18 months from the onset of estrogen treatment, and its best to wait until that growth has finished. In addition, the FDA has only approved the use of saline implants for women beginning at 18. You must be 22 to receive silicone implants per the FDA approval rules.

Ostensibly, its free across Canada, just like other medically necessary surgeries. But there can be unexpected fees in certain regions.
