Your Internal Bra Surgery

During your consultation with Dr. Basu, he will explain how the internal bra works in greater detail. He will also provide you with post-operative care instructions and a more specific idea of the results you can expect.

The internal bra technique is often combined with a limited scar periareolar lift or a full breast lift to tighten any loose skin on the breast. Depending on your individual case, the surgery can take anywhere from 3 to 5 hours. The internal bra is an outpatient surgery, so you can return home once your procedure is finished. In many cases, the internal bra can re-engineer a womans natural breast tissue to achieve results that a breast augmentation and lift alone simply cannot achieve.

Does Dr Moncrieff Use The Internal Bra Technique

Yes! Dr Moncrieff uses several variations of the internal bra technique when addressing breast implant revision, breast reduction and lift procedures. This technique is most useful for those with less resilience or elasticity in the skin who are hoping to achieve longevity in their surgical result, or those wishing to correct the breast implant complications as outlined above.

Want to know if the internal bra technique is right for you?

What Is Internal Bra

internal bra the new scary sounding boob job

The term internal bra is heavily used in the breast augmentation industry. Yet, not many people know that it refers to a myriad of different surgical techniques. Internal bra is actually an umbrella term including techniques intended to strengthen the lower pole of the breast. The idea of this technique is twofold. Firstly, to reinforce the inframammary fold. And secondly, to strengthen the lower portion of the breast. In tandem, this provides a more supportive foundation for a breast implant.

The most common technique used for internal bra is additional suturing. Sutures create a more secure and stable breast fold where the incision is. A rarer option used in more severe cases is the use of a surgical mesh. This mesh creates greater internal support of the breast implant. The internal bra technique can be used on breast lift or breast reduction patients if they are suitable candidates to prevent the breasts dropping.

What Is The Best Supportive Material To Create An Internal Bra

There have been dozens of materials developed with the intent of adding soft tissue support. Over the years, Dr. Yates has used many of these materials for both cosmetic and reconstructive indications. The gold standard for soft tissue support is ADM which has been made from either human or pigskin. Dr. Yates now prefers Galatea for most cases that require additional soft tissue support. Galaflex is made of poly-4-hydroxybutyrate . This material has many benefits to previous generations of surgical scaffolds.

  • Strength 3-4 x as strong as native tissue
  • It is entirely absorbed over time, leaving only supportive scar tissue.
  • Naturally broken down to CO2 and H2O by 18-24 months
  • Resistant to infection and seroma compared to previous materials

Why Do We Use Internal Bra

We all know that having a strong foundation for your breast implants is a good thing. But why exactly is it so important? One of the post-operative complications that can occur in patients is implant bottoming-out. Implant bottoming out refers to the displacement of a breast implant. This results in the implant sitting much lower in the breast than intended. Theres an important distinction between normal breast droop and bottoming-out. Breast droop is not a post-operative complication. It is however, caused by the patients natural anatomy changing over time. When breast droop occurs, the breast and nipple will move downward and create a drooping appearance. Bottoming-out on the other hand is when just the implant moves downward, and the nipple and natural breast tissue stay in place. You can read this blog post which includes a diagram of the difference between breast droop and bottoming-out.

Bottoming-Out

The internal bra technique strengthens the inframammary fold and lower-pole of the breast to help prevent bottoming-out after surgery. The additional support can create a stronger foundation for the breast implant.

Does Internal Bra Mean I Dont Have To Wear An Actual Bra

The term internal bra can be deceptive, as it suggests that the surgical technique is a substitute for physical support. The internal bra is an extra measure intended to reduce the incidence of bottoming out and breast droop, however there is no guarantee that this technique will provide enough support to prevent complications entirely. Physical support using a supportive bra or post-operative garment will play an integral role in maintaining results post-operatively and when combined with the surgical bra technique can help to ensure the best results.

Which Surgical Technique Is Better For Breast Lifting The Classic Anchor Technique Or Newer Lollipop Technique

Before and After Abdominoplasty 23 Gallery

The anchor breast lift is a very effective surgical treatment for breast uplifting. The name of the procedure comes from the shape of the incision used in the surgery: a nautical anchor.

Three incisions are made on the breast:

  • One around the areola in the shape of a circle
  • Incision two is made down from the base of the areola to the breast crease
  • And, the last incision is made along the breast crease, under the breast. This is the incision that is avoided when using the above Lollipop incision.

As mentioned previously, the lollipop breast lift involves only two surgical incisions: a ring around the areola and one straight incision from the bottom of the areola towards the breast crease. These incisions allow greater flexibility in adjusting the breast size and also make it easier to hide the cuts made on the skin. This technique is much more popular due to the fact that the scars are smaller.

The Laser Bra Procedure

The laser bra procedure is typically performed in a hospital or outpatient surgery center while you are under general anesthesia. Time in surgery is up to two hours.

The exact surgical details vary. For example, if laser bra surgery is part of a breast augmentation, your surgeon inserts the breast implants through an incision and positions them in the desired placement location. The doctor then applies energy from a carbon dioxide laser to the outermost tissue layer, creating an internal bra to support the remaining tissue and implant. The laser bra cant be seen or felt. It uses your own breast tissue to provide enhanced internal support.

Do Patients In Need Of A Breast Lift Need Internal Bras

The internal bra is intended to support a breast implant, not necessarily the overlying breast tissue. If a surgeon determines a patient has a unique situation where the implant support would be expected to be compromised, or in revision cases where an implant has bottomed out, Galaflex/ Galaform may be indicated as an internal bra. However, there are breast lift cases where an internal bra could be counterproductive over time.

Here is an example several years ago, textured implants were all the rage. The textured implants were designed to keep the implant in position, and they did a pretty good job of that. With this additional implant support, we saw more and more cases of breasts drooping over the implant. Particularly for women with heavy breasts without good natural skin support for their breast weight. The texture on the implant ONLY supported the implant, not the overlying soft tissue. A similar situation exists for an internal bra that supports the implant but could allow the overlying breasts to droop over the implant.

Preservation Of Nipple Sensitivity And The Ability To Breastfeed Trend #2

31) Exchange Surgery ⢠Expanders to Implants Fat Grafting ⢠Breast Reconstruction after Radiation

The nipple has two parts: the nipple and the areola. The nipple is attached to the milk ducts, allowing mothers to breastfeed their babies. Aside from minimizing scars, most women want to preserve the sensitivity of their nipples to ensure that they are still able to breastfeed should they want children in the future.

However, while there is no guarantee that the nipple will remain as sensitive as it was before the surgery, due to the development and studies of breast lift surgery, there is a higher chance of sustaining or keeping the sensitivity of the nipple.

How to preserve nipple sensitivity after surgery

One technique in keeping the preservation of nipple sensitivity would be to avoid the nerves below the nipple. The surgery ensures that the nipples are to be made a normal size, in symmetry, with good sensitivity so the patient should have no feeling of numbness or discomfort.

With meticulous surgical technique, it is achievable to retain the sensitivity of the nipplethough there is a possibility of numbness for some time during the healing process.

Once your breasts have healed, provided you have not had any complications your sensitivity should go back to normal.

Internal Bra Lift Vs Traditional Breast Lift

An internal bra lift and a traditional breast lift are not necessarily mutually exclusive. The internal bra support can be added to a traditional breast lifting technique to maximise durability.

The internal bra lift is considered to be more durable than traditional breast lifting. The inner tissue hammock provides extra support to the lower part of the breasts. It holds them up, counteracting the effect of gravity, and preventing them from sagging. This extra support at the inframammary line also helps prevent breast implants from bottoming out.

What Are The Benefits Of Using Surgical Scaffolds

The additional support provided by using these surgical scaffolds has innumerable benefits which include:

  • Preventing the double bubble appearance after breast augmentation, where an implant moves creating an unnatural look with two breast creases.
  • Prevention of symmastia, where the breast implants move, appearing to merge with no space between them.
  • Holds your breasts in the desired position for longer
  • Sustains the results of your breast surgery and allows for a more accurate estimation of the results
  • Slows the development of breast sag with larger implants or due to aging
  • Provides support for women with less elastic skin that may be prone to developing sagging breasts after augmentation

What Is The Internal Bra In Boca Raton

Before and After Abdominoplasty 14 Gallery

The internal bra involves a surgical procedure in which a supportive material such as a mesh-like matrix of fibers is inserted during breast surgery. The material is often attached to the chest wall. This mesh performs acts as an internal support system in 2 ways:

  • First, it counteracts the aging and breast sagging process by anchoring the breast tissue and skin in a fixed position.
  • Second, it acts as an invisible bra and provides a constant, hidden lift and support.

Dr. Farber offers two types of internal bras at his Boca Raton clinic: the GalaFLEX Mesh and an acellular dermis called Strattice.

The Internal Bra Defined

Over the past 17 years I have been working on the idea of an internal bra, and now that the idea is taking hold there seems to be some confusion about what it actually is. The purpose of a bra is to enhance the shape of the breast and add support that is lacking. In order to place this support within the breast, something has to be added otherwise it is just a traditional breast lift. Breasts that need lifting have sagged because the skin and ligaments that support the breast have thinned, stretched, and weakened, which is why all versions of breast lifting lose fullness in the upper pole over time they rely on weakened tissue.

Part of the misperception about internal bras relates to the patient who has implants that have dropped out of position and require a procedure to move them back up. Often this is done by suturing the scar capsule that forms around breast implants, a procedure called a capsulorrhaphy. But that is not an internal bra because it relies on tissue that has already shown that it is too weak for adequate support, just as in the case of breast lifts. A true internal bra is like an external bra but on the inside.
